Our Mission
DeGuardian Health Agency exists to make it possible for seniors and recovering patients across the Dallas–Fort Worth area to receive professional medical and personal care at home, where they are most comfortable and most likely to recover well. We believe that the best place to heal — and the best place to age — is at home, surrounded by the people and things that matter most.
Who We Serve
We serve families across Allen, Plano, McKinney, Frisco, Garland, Richardson, Dallas, Carrollton, Irving, Mesquite, and surrounding communities. Our clients range from post-surgical patients needing a few weeks of skilled nursing to seniors with advanced dementia requiring 24-hour care. Every care plan is built around the patient's specific medical needs, daily routines, and family preferences.
Credentials & Licensing
- Texas Health and Human Services Commission — Licensed home health agency
- Medicare-Certified — Qualified to provide skilled nursing, physical therapy, and occupational therapy under Medicare Part A and Part B
- Bonded and insured
- HIPAA-compliant — Patient information handled per federal privacy law
- All caregivers — Background-checked, drug-screened, TB-tested, and trained in CPR and safe patient handling
Texas HHS license number and Medicare provider number available upon request and listed in our service agreement.
Our Team
Our team includes registered nurses (RNs), licensed vocational nurses (LVNs), certified nursing assistants (CNAs), home health aides, physical therapists, and occupational therapists. Every caregiver receives orientation in our specific care protocols and is matched to clients based on the patient's medical needs, language preferences, and cultural background where possible.
How We Build a Care Plan
- Free consultation. We meet with the patient and family at home or by phone to understand needs.
- Physician coordination. For skilled services, we work with the patient's physician to obtain orders and certify medical necessity.
- Individualized care plan. Our nursing supervisor builds a written plan covering services, schedule, goals, and how progress is measured.
- Ongoing review. Care plans are reviewed every 60 days (Medicare requirement) and updated as needs change.
